Medical Certificate & Cardiopulmonary Clearance

MyHealth Clinic is ready to provide you with the certification necessary to ensure you are fit to work or cleared for surgery. Just the written approval of your consulting doctor and you can gain your certification immediately after payment, even for online consultations.

Disclaimer: The medical certificate only covers the following reasons:

1) Fit to (Resume) Work upon recovery from sickness
2) Advice to Rest/Medical Leave (as per doctor’s recommendation after proper evaluation)
3) Fit to Work after getting cleared of findings from Pre-Employment Medical Exam (PEME). This is applicable only to patients who had abnormal findings from their PEME, and were subsequently advised to undergo further evaluation and clearance from specific specialists.
4) Fit to Travel
5) Fit to Engage in Sports/Strenuous Physical Activities (e.g. run/marathon, swim, hike)
6) Supplementary medical certificates issued by MyHealth Clinic (if there be a need other than forms given by patients, like those for SSS, insurance, school requirement, proof of vaccination).

Purchases for medical certificates or cardiopulmonary clearances without prior approval from your consulting MyHealth Clinic doctor will be null and void.
